Who we are

Formwork is software that fitness studios use to run bookings, memberships and payments. It is operated by EpiHealth Solutions LLC, 11 Spring Rd, North Haven, CT 06473. Each studio is the business its members deal with: the messages a member gets, the classes they book and the payments they make are the studio’s, sent and processed through Formwork on the studio’s behalf.

What we collect

We collect what is needed to run an account at a studio and nothing we do not use.

  • Account details: your name, email address and a password (stored only as a one-way hash).
  • Contact details: your mobile phone number, if you add and verify one.
  • Booking and attendance records: classes you book, join a waitlist for, cancel or attend, and the credits and memberships you hold.
  • Payments: card details are entered directly with Stripe, on the studio’s own Stripe account. Formwork does not store full card numbers.
  • Text-message settings: whether you have turned text alerts on, when, and which version of the consent wording you saw.
  • Device and usage data: session cookies that keep you signed in, basic server logs, and data the installed web app needs to work.

How we use it

We use your information to:

  • create and secure your account and verify your email address and phone number;
  • book classes, run waitlists, and apply memberships, credits and fees under the studio’s policies;
  • take payments and issue refunds through the studio’s Stripe account;
  • send you the emails and, if you opt in, text messages described in our Terms;
  • keep the service running, prevent abuse and fix problems.

Who processes it

We share information with the providers that run the service, only so they can do that job:

  • Stripe, for payments;
  • Resend, for sending email;
  • our SMS provider, for verification codes and text messages;
  • our hosting and database providers.
  • The studio you belong to can see your account, bookings and payment status, because it is your studio.

Mobile numbers and text messaging

Mobile phone numbers and text-message opt-in data are not shared with third parties or affiliates for marketing or promotional purposes. They are shared only with the providers named above, to deliver the messages you asked for.

You can turn text alerts off at any time in your account, or reply STOP to any message.

Retention

We keep your information while your account is active and as needed for the studio’s and our records and legal obligations. You can ask the studio or us to delete your information; some records may be kept where we are required to keep them.

Your rights

You can ask us or your studio to:

  • show you the information held about you;
  • correct information that is wrong;
  • delete your information;
  • stop text messages (turn off text alerts in your account, or reply STOP).
